What is
North Hartford Ascend?
Facilitated by Connecticut Children’s, Ascend is an initiative which provides access to the children and families living in the North Hartford Promise Zone, to the services and supports they need to reach their full potential from prenatal-to-career initiative.
With the support of Ascend, affiliates will work to eliminate disparities in outcomes based on race, ethnicity, disabilities and other factors so children and families are able to thrive.

Between Jan 2022 & Dec 2023, Ascend reported data for 44 programs, services & events through 16 agencies. 93% of all active services included continuous programs.

Programs newly enrolled over 41K individuals in this time period. Most programs enrolled participants from more than one age group spanning from prenatal to 65+.

Between Jan 2022 & Dec 2023, over 38K individuals actively participated in programs. Estimated 17K individuals completed programs during this time period.

Referrals Made
Estimated 9,000 referrals were made to other programs and services. Majority of referrals were to programs in the health, safety and well-being domain.
